Medical DirectorDr. Kozarek has been Tara’s Medical Director since June 1996. He is a graduate from the University of Wisconsin Medical School. Licensure and Certification: Dr. Kozarek is a Board-Certified Family Physician. Professional Affiliations: Dr. Kozarek is a member of the Indiana Medical Association, American Academy of Family Physicians and the American Board of Family Practice. Dr. Kozarek is affiliated with the Johnson Memorial Hospital, Franklin, Indiana and the Columbus Regional Hospital, Columbus, Indiana.
